New Patients

At Synapse we want to make your first visit to our clinic as comfortable and stress-free as possible. We want to focus on "You" when you are here so that we can get to know you as a person and a patient. Getting the necessary "paperwork" out of the way beforehand helps both of us accomplish this goal.

This page has been designed to provide the first time patient with the necessary information and forms before your first visit. Our goal is for you to have a clear understanding on what you can expect during your first visit to the Synapse Clinic, no surprises!

This page is broken into sections to better organize the relevant forms and information guides. At a minimum, we will need you to complete Forms One, Two, and Three: "Patient Information Sheet", "Financial Policy",  & "Cancellation Policy"  and bring those to your first appointment. You may also preview all of the documents that are included in the "Welcome Packet" that you will receive at your first appointment.

To Download a Form, click on the form Title. After the document opens, use your browser's "Save As" feature to save a copy to your computer. You can also print the document directly from your browser.
